Aims/Background Safety plan and Goal setting are standard interventions offered by CAMHS Crisis Resolution Home Treatment team. The Department of Health Best Practice Principles NICE guidelines recommend collaborative production risk management plans recovery orientated input. We wanted to ensure compliance understand patient's views on usefulness these.Aims:- Did all patients complete safety goal setting? Was this done collaboratively?Did these strategies help?Were documents easy access? Methods There were two arms the project. 1. Review 40 electronic case notes out 715 seen from Jan April 2021. looked at completion plan, setting, goals achieved, HONOSCA scores, ratings mood scores. 2. Between July 2022, willing requested anonymised questionnaire through a QR code. asked if felt included when completing whether helped access. A free text option for feedback was also given. This proposal approved Improvement Knowledge Hub within LPT NHS trust who confirmed that ethical approval not required. Results reported in percentages. Case review (n=40): • 99% had completed, collaboratively. 90% collaboratively Rationales documented completing. 85% Improved scores improved 65% achieved 2 or more 3 set. 67.5% admission (average score 13.6) discharge score10.5) – improvement documented. Questionnaires: 18 questionnaires returned. 100% completed 83% they doing setting. said helpful recovery. 94% helped. 82% access documents.
